Summary


The Hammersmith & Fulham Community Independence Service (CIS) operates through a partnership between West London NHS Trust (WLT) and Central and North West London Foundation NHS Trust (CNWL). WLT employs staff in Hammersmith & Fulham, while CNWL serves as the lead partner for the service across Hammersmith and Fulham, Kensington and Chelsea, and Westminster boroughs. As the CIS Borough Lead, you will have a central role in overseeing and coordinating community-based pathways, including the management of the Community Independence Service in Hammersmith & Fulham.

The role involves overseeing service delivery pathways including Rapid Response, Rehabilitation, Home First, Case Management and also provide line management support to the PCN Paramedics based at North PCN at H&F.

We are seeking a candidate with strong drive, enthusiasm, and vision to lead the Hammersmith & Fulham Community Independence Service. The role involves overseeing service delivery pathways including Rapid Response, Rehabilitation, Home First, and Case Management, in addition to integrated working with partners in Adult Social Care Reablement. Key duties encompass ensuring service performance meets established KPIs, maintaining quality, safety, and effectiveness standards, managing the borough's operational budget, and overseeing workforce management. Collaboration with staff, partners, and stakeholders to identify opportunities for service improvement and innovation is essential.
