It provides care and treatment in the least restrictive environment resulting in minimum disruption to the Service User's life with the purpose of meeting the person's mental health needs in their home environment and providing a real alternative to hospital admission where possible.

The Team also facilitates early discharges from the inpatient wards and facilitates 48-hour post discharge telephone contact post-discharge from the acute wards, and 7-day follow up where appropriate.

The team base is at West Middlesex University hospital site, next to Syon Park and Old Isleworth with great connections into London. If appointed, you will be joining an experienced and welcoming team of consultants, who scored above benchmarks in our recent Staff Survey. We have a clear sense of purpose and strong sense of team.
