Summary


This role was established as part of the Community Mental Health Transformation, and is now a well established role within the Harlow North PCN and the wider Harlow mental health services, delivering a new and innovative approach to meeting the needs of Harlow residents at early stages of a mental health presentation.

You will be responsible for the clinical care support and treatment of patients with a mental health presentation within the Harlow North Primary Care Network (PCN).

Be an active and integral member of the PCN MDT contributing expert knowledge to the formulation and delivery of care and treatment plans.

The post holder will facilitate the management of flow and capacity at a primary care level and the interface with secondary care mental health and the voluntary sector.

The post holder will work with the wider health and social care economy including non-statutory and statutory organisations in the creation of integrated care plans to support people in their communities.


• Contribute fully to the integrated care agenda within the primary care networks.
• Engage in a systematic case discussion process with all PCN/community/Social Care staff.
• Produce accurate and timely documentation, conduct and manage the maintenance of accurate and timely documentation / records on all service users in line with your Professional Code of Conduct and Trust policy and the contractual requirements
• Conduct and supervise assessment and observation to ensure appropriate and timely feedback to the team
• Report and document incidents as required and act upon findings as appropriate
• Provide education and training packages to support and develop patients understanding of mental illness.
• Provide practical support, guidance and education for services users, carers and other professionals as required
• Lead and participate in formulation, delivery and review of individualised evidence based care / care packages according to NICE guidance including non-medication strategies
• Lead in the continuing development of a high quality service clinically underpinned by evidence based practice, care pathways, NICE and DH Guidance
• Understand the eligibility for Care Act Assessments and ensure appropriate assessments are undertaken.

• Undertake comprehensive outcome focused mental health assessments.
• Be competent in the use of relevant screening and assessment tools.
• To utilise the agreed patient outcome measure tool to support needs assessment and evaluation of effectiveness of interventions against agreed recovery outcomes.
• Undertake a range of short term interventions commensurate with assessed needs and in line with NICE and other appropriate National guidance inclusive of psychologically informed interventions.
• Work in collaboration with the VSCE mental health coaches providing supervision and oversight of their work.
• Work collaboratively as part of the PCN integrated mental health service.
• Participate in all appropriate PCN and specialist mental health multi-disciplinary meetings as required.
• Participate in the analysis and audit of integrated community pathways and make proposals to support sustainable management of community resources in consultation with integrated stakeholders.
• Support the PCN in the delivery of population health indicated services, interventions and approaches
• Ensure that services are accessible to all ensuring modes of delivery are flexed to enable access by all communities focusing on those who have historically been difficult to engage. Participate in and provide skills and knowledge training for the wider PCN roles in awareness, care and support of those with mental health needs
• The role will support the Health and Care partnership ambition to support people at an early stage of need working with local assets to facilitate “right, right place” support for people.
• Work with people to “stay well” utilising the whole PCN and local community asset.
• Work with colleagues in the specialist mental health community teams to support transition to the PCN integrated mental health services and to support transfer to the specialist teams when presentations become more complex.
• Work with colleagues in the development, implementation and monitoring and training of allocated care bundles in line with practice standards within the primary care networks.
• Undertake a leadership role with other PCN colleagues in the delivery and continues development of a high quality service that is underpinned by evidence based practice, care pathways/packages and NICE guidance.
• Where required provide leadership and day to day over sight of service delivery across the PCN and integrated neighbourhood teams.
• The service will initially work to a 5 Day, 9 – 5 rota however in line with future developments the service may be extended to providing a 7-day a week response or possible evenings
