Summary
This role is for a clinician to support primary care as part of our community care team working during the working day, Monday to Friday. The post holder will provide holistic care for individuals in their homes including assessment, diagnosis and management to provide the best outcomes for the patient. The post holder will have the ability and confidence to safely and comprehensively carry out a full systems assessment, autonomously discharging patients within your own scope of practice seeking senior colleague advice as required. It is important that you are comfortable working in this capacity though you will always have access to support from senior clinical colleagues at other sites. The postholder must be able to refer and/or signpost patients in their own home or care home to other allied health professionals for example community COPD nurses, Social prescribers etc.
