Job Title: Physiotherapist Grade: Band 6 Ward/Dept: Community Neuro Rehab Team Location: Mount Gould Hospital, PL4 7QD Trust: Livewell Southwest Hours: 22.5 hours Monday-Friday Shift Pattern: 8:30-16:30 Pay Rate: £21.80 An exciting opportunity has arisen for a Physiotherapist for the Community Neurology Rehabilitation Team based in Mount Gould Hospital. We are seeking a candidate who is able to operate within various community setting and provide advanced assessments of patients with neurological presentations. Livewell Southwest provides integrated health and social care services for patients across Plymouth, South Hams and West Devon, as well as parts of Devon and Cornwall. By integrating health and social care we are able to deliver the right care for people, in the right place and at the right time. You will find our teams across the community in hospitals, GP practices, sports centres, health, and wellbeing hubs. The post holder will be working within a multi-disciplinary team to deliver individualised treatment programmes through advanced assessments and undertake all aspects of clinical duties as an autonomous practitioner. This will include utilising investigative and analytical skills to formulate individualised management and treatment plans, using a range of communication to communicate with carers to gain their co-operation in order to progress patients rehabilitation and to operate as a lone practitioner when working in community and domiciliary settings with support of management via telephone. There will be a responsibility to provide supervision to junior colleagues, plus deliver expert advice, teaching and training to other members within the MDT regarding the management of neurological patients whilst working jointly with colleagues in a co-ordinated multi-disciplinary approach. To be successful in applying for this role you will need: Current relevant professional registration Minimum 2 years post-qualification experience Enhanced DBS UK/NHS experience Experience of working in a community setting Experience of working within Neurology Full UK license and access to own car for work purposes Job description Job description can be provided upon request.
