Summary


Looking for a new challenge, more flexible working hours that fit in with your work life balance than look no further! Our Community Nursing team in Wokingham, Berkshire are recruiting for District Nurses/ Community Nursing Sisters/Charge Nurses.

In this role you will provide support and leadership to a great team and join a team that provides a catalogue of care and pride themselves on their commitment to the well-being of the community. Ideally the candidate will bring with them clinical experience of working with adults in a community setting. Your working hours will be Monday to Friday, 8.30am to 4.30pm (with approximately one weekend every 4-5 weeks and bank holidays and some late shifts from 11.00am to 7.00pm on a rotation basis). We welcome applicants who would like to work part-time or on a job share. A full UK driving licence and the use of a car for work purposes is essential for this role.

We are seeking an enthusiastic and experienced nurse, with a warm personality and great people skills. You will be willing to undertake relevant training, have leadership experience and be able to demonstrate innovative practice. Show effective management skills, you will take responsibility for caseload management of housebound patients with multiple nursing need in the community. 

You will be responsible for ensuring high quality evidence-based care for patients, their families, and carers in the community.
• Responsible for a complex and diverse caseload, actively working to prevent admission to the acute sector and supporting patients in their preferred place of care. 
• Responsible for maintaining detailed, accurate and confidential patient records, adhering to Trust policies. 
• Leading an effective supportive team, working together to provide clinical advice and support to other staff and patients. 
• Responsible for the management of a defined caseload of patients, with continuing responsibility for the assessment of care needs, the development, implementation, and evaluation of care programmers. 
• Accountable for the standards of care and for determining the activity of your team. This will include acting as a role model for staff, sharing learning and developing practice.
