Summary

Joining the Transfer of Care Hub team as an Administrative Assistant, you’ll provide excellent administrative support and assist with the delivery of essential hospital discharge services.

What you’ll do:
• Manage the Transfer of Care Hub inbox
• Track referrals via our SharePoint list
• Transfer cases from Hospital Teams to Community Teams using our case management IT system
• Run reports and transfer data between our different trackers
• Take meeting minutes.

What we’re looking for:
• Microsoft Office 365 skills including Excel.
• Good written and verbal communication skills.
• Strong prioritisation and organisational skills.
• Resilience and adaptability.
• Strong customer focus to build relationships.
