About the Role

We have an exciting opportunity for a motivated and passionate CCS Practitioner (Complex Care) to join our Creative Care Solutions (CCS) team. This is a national role focused on developing, coordinating and supporting bespoke complex care packages for children and young people with highly individual needs.

Working closely with fostering agencies, clinical teams, Local Authorities and other professionals, you will play a key role in designing trauma-informed, child-centred and outcome-focused care solutions. You will support the assessment and matching process, contribute to risk management and safeguarding planning, and help ensure that children receive the right support to achieve positive outcomes.

Who We Are Looking For

We are looking for someone with an understanding of complex care packages with a trauma aware approach. You will have experience working with children and young people with enhanced and complex needs and be confident in analysing information, assessing risk and contributing to care planning. Ideally having experience of working in a referral hub environment to bring experience of working with LA's putting offers forward.

The successful candidate will be an excellent communicator with the ability to build positive relationships with internal teams, foster carers, Local Authorities and multi-agency partners. You will be highly organised, able to manage competing priorities and maintain accurate records while working in a fast-paced environment.
