About the Team and the Role

The Case Management Team supports older adults and adults with physical disabilities, including individuals with co‑existing mental health or substance misuse needs. We provide longer‑term (6 weeks or more), complex social work interventions with a strong focus on professional autonomy, multidisciplinary working, and strengths‑based, relationship‑centred practice.

Our practice model recognises that adults bring skills, resilience, networks, and lived experience that can be built upon to promote independence and wellbeing. You will work collaboratively with adults, carers, and community partners to identify strengths, support goals, and enable people to have as much choice and control as possible.

Safeguarding

You will lead and contribute to Care Act 2014 Section 42 safeguarding enquiries, working closely with partner agencies to assess, mitigate, and manage risk. You will apply professional curiosity, sound judgement, and a person‑centred, strengths‑based approach, ensuring adults remain involved in decision‑making wherever possible.

Mental Capacity Work

The team undertakes a high volume of complex mental capacity assessments. You will be expected to apply the Mental Capacity Act confidently, evidencing clear reasoning, robust analysis, and least‑restrictive decision‑making to support adults' rights and autonomy.

Court of Protection

Where required, you will prepare documentation for and contribute to Court of Protection applications, including matters relating to residence, care, contact, and safeguarding concerns. This requires practitioners who can navigate complex legal processes with professionalism, accuracy, and attention to detail.

Other Key Responsibilities

Undertaking Care Act assessments and reviews (including urgent and unplanned reviews), and coordinating multidisciplinary involvement in Continuing Healthcare processes.
Managing a carefully monitored caseload to ensure high‑quality, person‑centred interventions.
Building strong professional relationships with adults, carers, and partner agencies to support positive, strengths‑focused outcomes.
Applicant Requirements

Applicants should have:

3–4 years post‑qualifying statutory experience, ideally within community‑based adult social care.
Experience working with adults with complex needs, including safeguarding, mental capacity work, and multi‑agency practice.
Strong assessment, analysis, and report‑writing skills, with the ability to balance autonomy with collaborative working.
A confident understanding of strengths‑based practice, using asset‑focused approaches to promote independence, build resilience, and co‑produce meaningful outcomes with adults and carers.
