Summary

1 day in the office day and be flexible to come in extra days. Full time Monday-Friday availability
Full time OT role, Mon-Friday availability. Please note that CV from OTs without Social Care experience will be declined. Only send CV for OTs who have worked in Adult Social care and undertaken the work as listed in the JD
through major housing adaptations, manual handling assessments, and the provision of equipment and minor major adaptations. You will work closely with service users, carers, and partner agencies to deliver practical solutions that transform lives.
Key Responsibilities
• Complete comprehensive functional assessments in line with the Care Act.
• Take part in duty triaging and screening/signposting incoming referrals before accepting them to the waiting list
• Design and recommend major adaptations (e.g., level-access showers, stairlifts, ramps) to promote independence and reduce risks.
• Prescribe and arrange minor and major equipment to meet individual needs.
• Undertake manual handling risk assessments and provide safe moving and handling plans.
• Liaise with contractors, housing teams, and grants officers for adaptation works.
• Maintain accurate records and produce professional reports for funding applications and safeguarding processes.

Main Accountabilities
• Deliver high-quality, person-centred interventions within agreed timescales.
• Ensure compliance with legislation, safeguarding, and health & safety standards.
• Contribute to service development and continuous improvement.
• Work effectively within a multi-disciplinary team and represent the service professionally.
Mandatory Experience
• HCPC registered Occupational Therapist.
• Experience in community-based social care settings.
• Knowledge of housing adaptations, equipment provision, and manual handling.
Nice-to-Have Skills
• Familiarity with Disabled Facilities Grants (DFG) and adaptation legislation.
• Experience working with contractors and housing teams.
• Ability to use electronic case management systems.
