Summary

We are delighted to offer an exciting opportunity within our Family Hub's across the North of the Borough (Knutsford, Wilmslow, Macclesfield & Congleton) for two dedicated practitioner's to join our Start for Life Portage Service.

We are seeking 2 passionate and committed individual's who hold a full and relevant Early Years qualification at Level 3 (minimum) and have a genuine enthusiasm for supporting children with Special Educational Needs and Disabilities (SEND).

The successful postholder will have:

Experience of working in partnership with a wide range of practitioners, families and professionals to support children to reach their full potential
A strong understanding of early years programmes and early intervention approaches
The ability to support the delivery of high-quality services in line with the Best Start in Life Family Hubs programme aims and core offer

The role will involve working within Family Hubs with children and young people aged 0–19 years (up to 25 years for those with SEND). You will deliver home visiting sessions to support the home learning environment, providing tailored support to children with additional needs and their families, ensuring they receive effective early intervention.
