In this role, you'll play a vital role in supporting individuals with various needs. You'll collaborate closely with them to understand their goals and aspirations, developing personalised support plans that promote independence and well-being.
What You'll Do:
Be a Partner, Not Just a Carer: Work alongside individuals to understand their unique needs, preferences, and goals.
Empowerment Through Support: Develop and implement personalised plans that promote independence in daily living skills, from personal care and meal preparation to budgeting and household tasks.
Community Connections: Assist individuals in accessing and enjoying the community, including social events, leisure activities, and local services.
Mental Health Champion: Offer emotional support, encouragement, and strategies for managing mental health conditions.
Safety First: Identify and manage risks, ensuring the well-being of individuals at all times.
Communication is Key: Maintain accurate records and communicate effectively with colleagues, families, and other professionals involved in the individual's care.
Advocate and Champion: Be a voice for the individuals you support, ensuring they have access to the services and support they deserve.
Teamwork Makes the Dream Work: Collaborate effectively with a multidisciplinary team, including support workers, nurses, occupational therapists, and mental health professionals.
Who You Are:
You have experience supporting adults with mental health needs in a supported living or similar setting.
You possess excellent communication and interpersonal skills, allowing you to build rapport and trust with individuals.
You have a strong understanding of mental health issues and effective support strategies.
You can work independently and seamlessly as part of a team.
You're organised, adaptable, and can manage your time effectively.
You're committed to delivering person-centered care.
A relevant qualification in health and social care (e.g., NVQ Level 2/3) is a plus.
A full UK Driving Licence is desirable but not essential for this role.
