Summary
Job Purpose
To provide high-quality, person-centred support to adults attending the day centre, enabling them to participate in meaningful activities, develop life skills, increase independence, build social connections, and achieve their personal goals.
The Support Worker will assist individuals both within the centre and in the local community, promoting choice, dignity, inclusion, and wellbeing at all times.
Key Responsibilities:
Supporting Individuals
Provide person-centred support tailored to individual needs, preferences, and aspirations.
Encourage independence, choice, and decision-making.
Support individuals with communication, social interaction, and emotional wellbeing.
Build positive and professional relationships with service users, families, and professionals.
Support individuals with behaviours of concern using Positive Behaviour Support (PBS) approaches.
Activity Support
Support and facilitate centre-based and community-based activities.
Encourage participation in social, recreational, educational, and wellbeing activities.
Support service users to access community facilities, volunteering opportunities, and local events.
Assist individuals in developing life skills, confidence, and independence.
Promote inclusion and meaningful engagement throughout all activities.
Health, Safety and Safeguarding
Follow all safeguarding policies and procedures.
Report concerns, incidents, accidents, and near misses promptly.
Support safe working practices and contribute to a safe environment.
Assist with risk assessments for activities and community outings.
Maintain awareness of health and safety responsibilities at all times.
Record Keeping
Maintain accurate daily records and progress notes.
Record incidents, achievements, observations, and outcomes.
Contribute to reviews and support planning processes.
Maintain confidentiality and comply with GDPR requirements.
Team Working
Work collaboratively with colleagues, families, advocates, and external professionals.
Attend supervision, team meetings, and mandatory training.
Contribute positively to the development of the service.
Act as a positive role model for service users and colleagues.
Essential Requirements
Care Certificate (completed and in date).
Previous experience supporting adults with learning disabilities, autism, physical disabilities, mental health needs, or other special needs.
Positive Behaviour Support (PBS) training and practical experience.
PMVA (Prevention and Management of Violence and Aggression) training or equivalent.
Good understanding of person-centred support and safeguarding.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.
Good written and IT skills for record keeping.
Enhanced DBS check (or willingness to obtain one before employment commences).
Right to work in the UK.
Desirable Requirements
Level 2 or Level 3 Diploma in Health and Social Care.
Experience working within adult day opportunities, supported living, residential care, or community services.
First Aid qualification.
Experience supporting individuals with complex needs and behaviours that challenge.
