Job Title: Band 6 Mental Health Practitioner Trust: Hampshire & Isle of Wight Healthcare NHS FT Location: Saint Mary's Community Health Campus, PO3 6AD Ward: Adult Mental Health Pay Rates: £21.57 per hour Shift Patterns: Mon to Fri 9-5 (37.5 hour week) Why might this job interest you? We are seeking a dedicated Band 6 Mental Health Practitioner join the A2i Team in Portsmouth on a 6- month temporary contract. This is a full time position. This is an exciting opportunity to work within a dynamic multidisciplinary team, providing assessment, care coordination, therapeutic interventions, and recovery-focused support to service users in the community. As a Mental Health Practitioner, you will act as a Lead Professional for a caseload of individuals with complex mental health needs, ensuring the effective implementation of the Care Programme Approach (CPA). You will work collaboratively with service users, carers, and partner agencies to promote recovery, independence, choice, and personalised care. You will provide comprehensive assessments, develop and review care and risk management plans, facilitate multidisciplinary meetings, and deliver evidence-based interventions that support positive outcomes. In this role you will be required to: Manage a caseload of adults with severe and enduring mental health conditions. Undertake comprehensive assessments, including risk assessments and care planning. Act as Lead Professional and coordinate care in line with CPA requirements. Deliver recovery-focused therapeutic interventions and support plans. Work collaboratively with service users, carers, and partner agencies. Chair and participate in CPA reviews, MDT meetings, case conferences, and professional meetings. Maintain accurate and timely electronic clinical records. Support, supervise, and mentor junior staff, students, and trainees. Contribute to service development, quality improvement initiatives, audits, and clinical governance activities. Promote safeguarding, equality, diversity, and person-centred care at all times. Deputise for the Clinical Manager when required. What youll need: Degree-level qualification or equivalent professional qualification. Registered Mental Health Nurse (RMN), Social Worker, or Occupational Therapist with current professional registration. Experience working with individuals with severe and enduring mental illness. Experience implementing the Care Programme Approach (CPA). Knowledge and experience of risk assessment and risk management. Experience working within multidisciplinary and multi-agency environments. Ability to work autonomously, prioritise workload, and manage a caseload effectively. Competent IT and clinical record-keeping skills. Full UK driving licence and ability to travel across service locations.
