Our team is built around the individual needs of the people we support, and we are now looking for an experienced and dynamic Clinical Lead Nurse to join us and help lead our service forward.

The Role
This post is for an experienced and qualified nurse with advanced clinical and leadership skills. You will lead, coordinate and support our established care team in the provision of high quality care for clients in their own homes, including those living with dementia and complex needs.
You will be expected to coordinate complex care packages, oversee assessment and management of long-term conditions, and apply strong case management skills to promote independent living, self-management of health, and reduction of hospital admissions.
The focus of this role is to provide safe, effective and quality clinical care, using advanced qualifications to support clinical practice alongside strong leadership, advice and guidance to the wider team.
Key Responsibilities
- Lead, coordinate and support the care team in the delivery of person-centred, high quality care
- Manage complex cases and long-term conditions including dementia and TDDI
- Oversee care assessments, planning and reviews
- Provide staff supervision, appraisals and development, including support for junior staff
- Act as educator, mentor and preceptor to the team
- Ensure compliance with CQC standards and promote best practice
- Liaise effectively with the wider multi-disciplinary team
- Champion self-care management principles using innovative patient activation strategies
- Reflect upon and where needed challenge the status quo to drive continuous improvement
Person Specification
Essential:
- Qualified Registered Nurse (RGN/RMN)
- Extensive clinical experience including complex case management
- Experience in domiciliary or community care
- Knowledge and experience of dementia care and TDDI
- Evidence of ability to maintain and monitor high standards of care
- Strong leadership, communication and negotiation skills
- Ability to work autonomously, analyse complex situations and manage risk
- Understanding of clinical governance and CQC standards
- Prescribing qualification or willingness to work towards
- Practice assessor/mentorship qualification
- Advanced Physical Assessment qualification
- Continuing Professional Development evidence
- Car driver with access to a vehicle for work purposes
Desirable:
- MSc or equivalent
- Leadership qualification
- Community Specialist Practitioner (CSP) qualification or working towards
