Summary


Hawthorn Intermediate Care Unit is a Dynamic and Exciting Environment offering Patients the optimum Physical Rehabilitation and Nursing Care available.

Hawthorn is a Intermediate Care Unit which is located within the Grounds of Hillingdon Hospital, it has a bright, airy sense with its high ceilings within the Atrium of the Unit giving a fresh bright outlook.

Hawthorn is a Nurse Led Unit comprising of Ward Matron, Ward Manager, Ward Sisters, Registered Nurses, Health Care Support Workers, Therapy Lead Manager, Physiotherapists, Occupational Therapist, Therapy Technician, Doctor, Pharmacist & Care of the Elderly Consultant.

We work as MDT delivering the best care to all of our patients, offering them patient centred care with an excellent clinical/therapy outcome.
• Delivering patient centred care
• Delivery personal care
• Safeguarding discussions
• Toileting Patients
• Assessing patients skin
• Taking Vital Signs
• Handover using SBARD Tool
• Assisting feeding (when necessary)
• Reviewing patients Nutritional & Hydrational Needs
• Making Beds & Bed Movement
• Using Mobility Aids to transfer patients
• Escorting patients to appointments
