Located near Savile Park in Halifax, Heatherstones Court is a welcoming reablement and specialist housing service supporting people from 18 years of age and above to regain confidence and independence. Our modern setting includes eight single apartments and three double apartments, providing a safe and comfortable environment where people can receive high quality primary care during their stay.

We’re looking for a dedicated and compassionate Reablement Support Worker with proven experience in medication good practice to join our friendly, supportive team.

Heatherstones Court provides support for:
• People on discharge from hospital who need a short period of help and support (Reablement) before getting back to their former self.
• People who are experiencing difficulties in their own home and can no longer access their home because of changes to their health or personal circumstances.
• People who have become homeless because they are unable to return to their own home.
• People may be living in a property which requires urgent repairs or has been assessed as unsafe to reside.



About the Role

We believe that with the right support, encouragement, and guidance, individuals can regain their confidence, mobility, and daily living skills to live fulfilling and independent lives.
Our team of dedicated and compassionate care professionals are trained to provide personalised care plans that focus on everyone’s specific needs, goals, and preferences. Through a holistic approach to care, we aim to address not only the physical aspects of reablement but also the emotional and psychological well-being of the people we care for.

As a Reablement Support Worker, you’ll work closely with individuals recovering from illness, injury, or hospital stays, supporting them to rebuild skills, confidence, and independence in their own homes. You’ll:

• Encourage and guide people to carry out daily living activities
• Support safe and accurate medication administration in line with best practice
• Promote wellbeing, dignity, and choice always
• Work collaboratively with healthcare professionals, families, and carers

Our Requirements
• Experience supporting individuals who require care and support

• Strong communication skills, with the ability to work effectively both independently and as part of a team.
• Relationship building skills and the ability to establish trust with people and colleagues.
• A flexible approach to work rotas is required, as we operate a 7-day service shift patterns are from 8 am until 22.00 hrs

• Experience in reablement, domiciliary care, or a similar support role
• Strong knowledge of medication handling, storage, and administration
• Strong written & verbal communication skills
• A caring, patient, and empowering approach
• Flexibility to work shifts, including evenings and weekends
