Summary
Deliver high-quality, patient-focused clinical care in line with current best practice Work effectively as part of a multidisciplinary team, including nurses, pharmacists, HCAs, mental health practitioners and social prescribers, to provide coordinated and holistic care Undertake patient consultations, including assessment, diagnosis and ongoing management Support the management of long-term conditions, alongside health promotion and preventative care Contribute to continuous improvement within the practice through participation in clinical audit, education and quality improvement initiatives
