Summary

Our Hospice Practitioners provide care for patients on our in-patient unit and also support our community-facing Hospice@Home service. Based at the hospice, our Hospice Practitioners will work with the existing team of experienced Advanced Clinical Practitioners, Resident doctors and Consultants in Palliative Medicine to ensure patients receive responsive and high quality care. Main duties and responsibilities Professional 1. Provide medical input and clinical care as required to Hospice patients who are either receiving care on the IPU or in the community, alongside the multi-disciplinary team. 2. Provide support and advice to community and hospital specialist palliative care teams and other professionals. 3. Liaise with other healthcare professionals to plan and deliver treatment in keeping with the patients wishes. 4. Actively participate in multi-disciplinary team discussions and meetings. 5. Maintain and administer medical records as per Hospice policy. 6. Be involved in audit, research and quality improvement programmes as appropriate. Managerial 1. Work closely with the Medical Director to ensure provision of medical input throughout the Hospice. 2. Support the development of clinical governance, including evidencing a commitment to clinical audit, risk management, staff development, within the Hospice governance framework. 3. Assist in delivering undergraduate teaching for medical student placements in the hospice. Additional Information 1. The post-holder must participate in the on-call IPU rota. When on-call, the post-holder will have consultant cover as a second tier. On call is currently 1 in 6 and is non-resident provided they remain within a 45-minute drive from the premises. 2. Doctors provide prospective cover for each others leave. Additional weekday and on-call sessions worked to back-fill other forms of leave attracts sessional payment at the usual rate.
