Summary
This is a permanent contract 18.75 hours with a 2 weeks rota as detailed below. Week 1 Monday - 9-1245 (Featherstone) Wednesday - 9-5pm (Featherstone) Friday - 9-5pm (Selly Park) Week 2 Wednesday - 9-5pm (Featherstone) Thursday - 9-1245 (Featherstone) Friday - 9-5 pm (Selly Park) We are a Federation that serves approximately 450,000 patients across over 60 GP Practices and 9 Primary Care Networks. This role is pivotal to improving quality of care and operational efficiencies which requires motivation and passion to deliver an excellent service within general practice as outlined in the NHS Long-Term Plan 2019 and GP Contract 2019. The role will involve working on central Federation services and services within Primary Care Networks and their practices. Person SpecificationEssential Structured Medication Reviews (SMR) Long-term conditions (LTCs) management Patient-facing role Care Home Medication Reviews Complex Polypharmacy management Elderly and frail patients care End of Life Care management and pathway (QOF) Quality Improvement Prescribing Indicators (QOF) Learning disabilities or autism reviews (Stop Over Medication Programme, STOMP) Specialist expertise of medicines Lead on person-centered Medicines Optimisation Lead on local Anti-Microbial Stewardship (AMR) within the PCN QOF contribution and Enhanced Services Lead on integrating Pharmacy teams across Primary Care, Community and Hospital as part of the wider healthcare team to improve patient outcomes Central role in clinical aspects of Shared Care protocols (ESCA), Specialist Drugs Checklists (RiCADs), high-risk drug monitoring, and clinical research of medication Qualifications Essential MPharm Mandatory GPhC registration Evidence of recent management and leadership development (e.g. Mary Seacole) Post-graduate clinical pharmacy qualification (e.g. clinical diploma, MSc, other post-graduate clinical modules) or equivalent experience Independent Prescribing Qualification Willing to enrol and complete CPPE Primary Care Pathway (18-month training programme), if not already completed Willing to consent to apply for enhanced DBS disclosure Minimum 2-years post-registration experience in Primary Care Pharmacy sector or 4-years in any other sector of Pharmacy Desirable RPS Member ACP (Masters in Advanced Clinical Practice) Familiarity with GP Clinical IT Systems Experience of effective partnership and working with internal and external stakeholders COVID-19 vaccination remains the best way to protect yourself, your family, your colleagues and of course our patients from the virus when working in our healthcare settings.
