Summary
Main Responsibilities Deliver safe, effective nursing care to patients presenting with urgent care needs in community, clinic, or domiciliary settings. Undertake holistic patient assessments, including physical observations, clinical history, and risk assessments. Monitor, record, and escalate physiological observationsin accordance withescalation protocols. Recognise clinical deterioration and follow escalation pathways Perform venepuncture and collect blood samples following procedures and infection prevention standards. Provide catheter care, including insertion (if trained), maintenance, troubleshooting, catheter changes, and patient education. Deliver basic wound care, including assessment, dressing selection, treatment planning, and ongoing evaluation of wound healing. Respond appropriately to patient falls, carrying out post fall assessments, neurological observations (where indicated), and escalation/referral as required. Use the lifting equipment after training Undertake verification of expected death in line with local policy, national guidance, and scope of practice, ensuring sensitive communication with families and carers. Undertake any other duties appropriate to the role and level of responsibility, as required to support the Urgent Access Primary Care service.
