Responsibilities
Conduct comprehensive assessments of adults with learning disabilities to identify their needs and strengths.
Develop and implement individualized care plans in collaboration with clients, families, and multidisciplinary teams.
Advocate on behalf of clients to secure appropriate services and support within the community.
Provide ongoing case management and monitor progress to ensure positive outcomes for clients.
Facilitate access to health, social, educational, and employment resources relevant to clients’ needs.
Work closely with families and caregivers to offer guidance, support, and education.
Maintain accurate and up-to-date client records and documentation in compliance with organizational standards.
Requirements
Qualified Social Worker registered with the appropriate professional body.
Demonstrated experience working with adults with learning disabilities or similar vulnerable groups.
Strong knowledge of social care legislation, policies, and best practices related to learning disabilities.
Excellent communication, interpersonal, and advocacy skills.
Ability to work independently and as part of a multidisciplinary team.
Proficiency in conducting assessments and developing person-centered care plans.
Commitment to continuous professional development and reflective practice.
Hours Per Week: 37.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £25.00
