8 Posts - Full Time Fixed Term Positions for 12 months due to funding.
If you are caring, motivated, positive and honest, this is an excellent opportunity to join our established Supporting Independence Team and make a real difference to people’s lives across the city of Hull. Our service plays a vital role in the discharge to assess model introduced by NHS England, which ensures that individuals who are medically ready to leave hospital can return home or move into a short term step down setting without delay. By providing short term care, rehabilitation and reablement, we help people recover in the right environment and assess their longer term needs once they have reached their optimal level of independence.
As a Support Assistant, you will work with adults who have recently been discharged from hospital or an integrated care facility, supporting them in their own homes to regain confidence and independence. No two days are the same, and the support you provide will vary depending on each person’s individual needs. This may include personal care, help with medication, support with meals and nutrition, and assistance with day to day activities. At the heart of the role is a commitment to safeguarding vulnerable adults and treating every person with dignity and respect.
We are a large community team covering the whole of Hull, so you will need to be flexible, reliable and willing to travel across the city to ensure people receive timely and effective support. Our aim is to help individuals live as independently as possible with minimal long term intervention, reflecting the Council’s vision of “a life not a service.”
You do not need a qualification to apply. However, you will be supported to achieve the Care Certificate and a Level 2 qualification in Health and Social Care within two years of joining us. You will receive full induction training, along with ongoing learning and development opportunities throughout your employment.

ROLE AND PURPOSE
The Adult Social Care vision in Hull is of ‘a life not a service’. Adult Social Care is person centred and focuses on peoples’ strengths to support them to take control of improving their own health, resilience, and wellbeing. We want to maximise people’s independence so they can achieve their goals and aspirations and live life to the fullest. Support Assistants work with people, their informal carers, local communities, and our partners, to support people on their active recovery and reablement journey to achieve their goals.
PRINCIPAL ACCOUNTABILITIES:
1. To promote and safeguard the welfare of children, young people and/or vulnerable adults.
2. Statutory Obligations - To implement each person’s individual care and support plan and risk assessment, to ensure their needs are met and identified outcomes achieved, and that any changes are recorded and reported to the Support Organiser on duty.
3. To attend to the physical and emotional needs of people in receipt of services, including dressing, washing, and bathing, toileting and other personal care and support needs providing dignity and choice.To encourage, support and advise people in receipt of services to undertake personal care and practical tasks for themselves. To include giving advice and encouragement to assist people to perform daily living tasks for themselves to promote independence and achieve their individual reablement goals.To provide encouragement and support to informal carers contributing to the care of people by signposting and raising awareness to partner agencies who can assist with enabling independence.
4. To undertake as necessary practical tasks including bed making, duties of a domestic nature, support people in receipt of services with shopping and laundry tasks, support with the preparation of meals and light snacks, drinks, and support people when required to eat and drink.Aid people to maintain independent use of their household budget through payment of bills, collection of benefits and pension. All necessary records of financial transactions to be made in accordance with Council policy and procedures.
5. To provide care and support to people in receipt of services with stoma and catheter care where this is an assessed need and have undertaken appropriate training. To support, monitor and report on people’s moving and handling requirements, reporting on changes, and communicating with line manager, requests for Occupational Therapist intervention or where it is identified a review of care and support needs is required, highlighting where there are concerns for health deterioration or where the person has achieved their active recovery goals.
6. To support and promote people as identified in their care and support plan to self-medicate or where necessary to administer medication as prescribed by the medical practitioner and maintain appropriate records according to departmental procedures and in line with the Universal Medication Policy. To be responsible for the reordering of medication as identified in the Fullers Assessment and to ensure the timely collection, delivery, and disposal of medication. To be responsible for checking the correct amount of medication has been issued to the person, and count medication when required to support with the checking of accurate medication management within the home of a person in receipt of services where the Fullers Assessment identifies SIT to be responsible.To be responsible for assessing the persons situation regarding self-administration and raise concerns for risk to Support Organiser on duty, family members and health practitioner.
7. To undertake administrative tasks maintaining up to date, accurate, quality data on manual and electronic data bases with reference to case recording.Recording, and reporting of dietary requirements, completing dietary and fluid intake charts, completing, and reporting on financial transaction forms and other relevant information in adherence with the General Data Protection Regulations.Ensure accurate completion of accident forms, incident forms and witness statements to adhere to HCC reporting procedures.
8. Work collaboratively with colleagues, managers and other health and social care professionals in line with Hull City Councils Competency Framework to support the city plan in achieving objectives to improve the lives of people we support. Demonstrating a flexible and adaptable approach to the working environment including utilising various communication tools and techniques, using IT equipment, face to face verbal and non-verbal communication, lone working in a variety of locations city wide. Support when required with market pressures and ensure the most vulnerable people in receipt of care and support are provided with the service they need within the place they call home, including those that do not have an active care and support plan. Be responsive in an emergency when there has been a request for lifeline support. Work cohesively with new colleagues enabling shadowing to share skills, knowledge and experience ensuring a supportive team approach and network.
9. The Health and Safety at Work etc. Act 1974 and associated legislation places responsibilities for health and safety on Hull City Council, as your employer and you as an employee of the council. In addition to the Councils overall duties, the post holder has personal responsibility for their own health, safety and wellbeing and that of other employees; additional and more specific responsibilities are identified in the Council’s Corporate HS policy.
