Summary
To support mental health service users in reducing their health risks and monitoring their health and to engage service users in considering their well- being through lifestyle adjustments. To monitor and report on as required infection control issues within the community team as directed by the team manager.
To support the smooth running of the physical health and wellbeing element of the community service. The post holder will conduct / offer. Venepuncture ECG Blood glucose monitoring Lifestyle advice Annual physical health monitoring The post holder will report to care coordinator / lead clinician and GP any physical health concerns that may require further investigation or intervention Post holder will contribute to meeting the trusts annual physical health targets and any other targets set by the physical health or infection control.
