Summary
Please see the attached job description for more information Manage referrals and discharges from the hospital and help assist with the co-ordination of them in line with national and locally agreed standards and guidelines Ensure queries from patients and their families are responded to in a timely and appropriate way To ensure that all discharges are facilitated in accordance with Trust and local policies To escalate any capacity or performance concerns to senior colleagues in a timely manner. To provide training and support to new and ongoing IDS team in various roles. Responsible for completion of various reports and returns in line with policy and procedure ensuring data integrity and professional standards are adhered too
