Summary
The post holder is an experienced General Practitioner who acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice and their own homes. They will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing clinical leadership as required. Key Responsibilities 1. Clinical Duties . Provide high-quality primary care to registered patients, diagnosing, treating, and managing health conditions. Perform clinical assessments, physical examinations, and appropriate investigations. To prescribe safe, effective and appropriate medication as defined by current legislative framework and ICB prescribing policies To monitor treatments, and review patients as required. Provide continuity of care, including chronic disease management and preventative health care. Participate in child health, family planning, and immunisation programs. Provide on-call cover Home visits as required To provide clinical leadership within the practice and support fellow clinicians To manage own referrals, test results and administrative duties in a timely manner To contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery 2. Patient Management Maintain accurate patient records using appropriate clinical systems. Communicate effectively with patients, families, and carers to ensure understanding of treatment and care plans. 3. Team Collaboration Work effectively as part of the multidisciplinary team including nurses, healthcare assistants, and administrative staff. Attend and contribute to team meetings, clinical reviews, and audit activities. To provide administrative cover for GP colleagues when required 4. Quality and Compliance Follow practice protocols and clinical governance guidelines. Ensure compliance with NHS and CQC standards for patient care. Participate in quality improvement initiatives and appraisals. Follow Cambridgeshire & Peterborough Medicines Optimisation Team (MOT) prescribing guidelines 5. Professional Development Engage in continuing professional development (CPD) and stay up-to-date with medical advancements. Maintain membership in relevant professional bodies and complete all requirements to maintain registration with the GMC. Complete professional appraisal as required Maintain all mandatory training as determined by the Practice PROFESSIONAL ROLE: The post-holder will : Promote evidence-based practice through the use of the latest research-based guidelines and the development of practice-based research. Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of audit and peer review Maintain their professional registration Participate in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained Work within the latest GMC Code of Professional Conduct Record accurate consultation data in patients records in accordance with the latest GMC/ CQC guidance and other pertinent standards Keep up to date with pertinent health-related policy and work with the practice team to consider the impact and strategies for implementation Work collaboratively with colleagues within and external to the practice. Demonstrate leadership Act as a mentor if required for clinical colleagues and through this, support colleagues personal development. Participate in clinical supervision Encourage and develop teamwork within the practice. Participate in multi-disciplinary protocol and Patient Group Directions development Help the practice operate in a cost-effective manner. Identify and manage clinical care risks on a continuing basis. Participate in practice meetings as appropriate. Participate in audits and inspections as appropriate. Work closely with the doctors, nurses and administrative managers in the setting up and/or improving of practice systems for monitoring/measuring performance against Clinical Governance and Quality Indicator targets. Ensure that all Practice Policies are fully implemented HEALTH AND SAFETY/RISK MANAGEMENT The post-holder must comply at all times with the Practices Health and Safety policies, in particular by following agreed safe working procedures and reporting incidents using the organisations Incident Reporting System. The post-holder will comply with the Data Protection Act (1984) and the Access to Health Records Act (1990). 6. BUSINESS CONFIDENTIALITY The post-holder will keep all information regarding the business, the partners and staff confidential and not disclose details to any third party. 9. GDPR The post holder must follow all GDPR regulations. 10. COMMUNICATION & WORKING RELATIONSHIPS The post-holder will establish and maintain effective communication pathways with the multidisciplinary team internally and externally. 11. REHABILITATION OF OFFENDERS ACT 1994 Because of the nature of the work, this post is exempt from the provisions of Section 4(2) of the Rehabilitation of Offenders Act 1994 (Exceptions) Order 1995. JOB DESCRIPTION AGREEMENT This job description is intended to provide an outline of the key tasks and responsibilities only. There may be other duties required of the post-holder commensurate with the position. This description will be open to regular review and may be amended to take into account development within the Practice. All members of staff should be prepared to take on additional duties or relinquish existing duties in order to maintain the efficient running of the Practice. Essential Qualifications/ Registrations and Skills Full registration with the General Medical Council (GMC). Entry on the Performers List Entry on the GMC GP register Membership of the Royal College of General Practitioners (MRCGP) or equivalent. Experience in primary care or general practice. Strong diagnostic and decision-making skills. A commitment to patient-centred care and clinical excellence. Excellent communication and interpersonal abilities. Desirable Qualifications and Skills Additional training in areas like minor surgery, family planning, or chronic disease management. Experience with specific clinical software (e.g., EMIS or SystmOne). Other Requirements Enhanced DBS clearance. A requirement of this post is to complete home visits when required; therefore the post-holder must hold Business Car Insurance, plus current MOT and tax for their vehicle.
