CLCH Hertfordshire Adult Community Services- CLCH NHS Trust provides a range of Community Services, and theHertfordshire Divisionis recruiting!

The Community Palliative Care Clinical Nurse Specialist (CNS) will join an established integrated Palliative Care team that works across organisational boundaries. This team supports patients with a variety of cancer and non cancer diagnoses, alongside long term conditions. The post holder will be a member of the Central london Community healthcare NHS trust Specialist Palliative Care Service. The CNS will be required to influence patient care by undertaking:


· Advanced Clinical Practice

· Training and Education

· Service Development/Leadership

· Research and Audit


The advanced clinical practice component will be achieved by providing specialist palliative care to an aligned patient caseload in partnership with primary health care teams and other specialist and generalist services. The CNS will be expected to contribute to the wider cancer and palliative care agenda, ensuring a flexible responsive service that meets the local needs within CLCH in Hertfordshire.

This post is for band 7 and band 6

· Provide highly specialist palliative care advice and support to other generalist nurses, care agencies and health professionals including doctors and consultants, to ensure high quality individualised patient care.

· Use expert skills in communication, clinical judgement and management within palliative care settings.

· Use and develop specialist skills when dealing with verbal and physical aggression whilst working with patients and carers.

· Function effectively as an autonomous practitioner working independently not directly supervised and as a member of a multidisciplinary team.

· Assess and manage a caseload of complex patients alongside other colleagues to ensure patients, carers and professionals are supported in specialist palliative care needs
