Summary
Job Purpose To provide high-quality, patient-centred nursing care within the practice, supporting the delivery of primary care services, chronic disease management, health promotion, and treatment room services. The post holder will work collaboratively with the multidisciplinary team to ensure safe, effective, and efficient care for the practice population. Key Responsibilities Clinical Duties Deliver high-quality, evidence-based nursing care to patients within the practice setting Lead and contribute to chronic disease management clinics (e.g. diabetes, COPD, asthma), in line with local and national guidelines Undertake cervical cytology (smear tests) in accordance with national screening programmes Administer immunisations and vaccinations in line with current protocols and Patient Group Directions (PGDs) Provide wound care, dressings, and a full range of treatment room services Assess, plan, implement and evaluate patient care Health Promotion & Patient Education Promote health and wellbeing through patient education and lifestyle advice Support national and local health promotion initiatives Encourage patients to engage in self-care and preventative health programmes Infection Prevention & Control Act in accordance with infection prevention and control policies Maintain a safe clinical environment for patients and staff Participate in audits and compliance activities where required Administrative & Record Keeping Maintain accurate, contemporaneous, and legally compliant clinical records Use clinical systems (e.g. EMIS) effectively to support patient care Contribute to data collection and quality improvement initiatives (e.g. QOF) Team Working & Communication Work collaboratively with GPs, nurses, healthcare assistants and the wider multidisciplinary team Communicate effectively with patients, carers, and colleagues Participate in practice meetings and contribute to service development Professional Responsibilities Maintain active NMC registration and adhere to the NMC Code Engage in continuing professional development (CPD) Participate in appraisal and revalidation processes Work within own scope of competence and seek support when requiredTraining & Development Participate in a structured induction programme Engage in ongoing training including chronic disease management, cytology, and immunisations (if required) Access mentorship and clinical supervision Take part in internal and external CPD opportunitiesGeneral Responsibilities Comply with practice policies and procedures Adhere to confidentiality, data protection, and information governance requirements Support equality, diversity, and inclusion within the practice Contribute to a positive and supportive working environment
