Summary
Job Purpose This District Nurse role will support and address the needs of both the professional and the leadership agenda of community services. The role is for a specialist nurse who will have direct responsibility for a complex caseload whilst providing specialist training, advice, mentorship teaching and support to Nurses and their teams. Leading on complex clinical caseload including long term conditions and multi pathologies. Providing specialist clinical advice to their teams using evidence based research and practice. Providing clinical supervision and mentorship to Community Nurses. Providing professional support to the management and leadership delivery of the service. To work collaboratively and across organisational boundaries specifically in collaboration with other local District Nursing employers and partner organisations. To provide a structure of ongoing clinical supervisory support for band 6 Nurses and all members of the Community Nursing team. To take shared accountability with the Heads of Service / Team Manager for the financial resources and workforce resourcing of the community services. Working in a number of locations across the City of Nottingham. Key Responsibilities Clinical Practice 1. Utilises highly developed specialised and advanced clinical knowledge covering a range of procedures and underpinned by relevant broad-based knowledge, experience, and competence as a qualified District Nurse. 2. Where appropriate to service delivery, demonstrates highly developed physical skills requiring accuracy and dexterity e.g., moving and handling techniques, clinical procedures, diagnostic tests. 3. Demonstrates independent practice responding to social, scientific, clinical, and ethical issues which are encountered. 4. Makes complex judgements requiring analysis, interpretation and comparison of options by integrating complex knowledge sources in new and/or unfamiliar contexts. 5. Responsible for complex clinical pathways. 6. Engages and actively involves the individual and, if appropriate, their family/carer, in the assessment, planning, implementation and evaluation of programmes of treatment and/or care by using person-centred techniques to promote a culture of self-care and empowerment. 7. Works to standards of proficiency identified by their professional and regulatory bodies performing clinical treatments/procedures to a highly specialist standard. 8. Prioritises their own workload and that of the wider team within agreed objectives deciding when to refer to others as appropriate. 9. Line management responsibilities as leaders of their team and delegation as appropriate. 10. Provides and receives complex, sensitive or contentious information where motivational, persuasive, empathetic, negotiating and reassurance skills are required whilst demonstrating an understanding of barriers to communication. 11. Approaches each individual with care, compassion and sensitivity ensuring that these values are reflected in all aspects of the role including the management of complaints and compliments. 12. To comply with the organisations Infection Prevention and Control requirements. Leadership 1. Provides a leadership style which is underpinned by strongly held values around equality, diversity inclusion and openness; effectively builds and maintains relationships with direct reports, management team and other key individuals across the organisation. 2. Ensures that a professional service and positive image for CityCare is maintained at all times, thereby acting as a role model to all staff. 3. Works collaboratively with Heads of Service and Team Leaders in the leadership and development of community services. 4. Works collaboratively with the Director Nursing and AHPs. 5. Ensures the effective and efficient use of resources e.g. staff, supplies within their own sphere of responsibility. 6. Demonstrates leadership and innovation in contexts which are unfamiliar, complex, and unpredictable that require solving problems involving many interacting factors. 7. Demonstrates the ability to delegate, organise and prioritise workload to ensure the safe delivery of the service including the management of referrals and discharges utilising locally agreed support mechanisms e.g. on call manager and making judgements in complex situations. 8. Demonstrates resilience when responding to challenge, change and complex or difficult situations. 9. Demonstrates clinical leadership, initiative and creativity in developing projects inspiring others to be positive in their contribution to continuous improvement. 10. Clearly articulates the expected clinical standards of the service, monitors and identifies where standards are not met and takes action to address. 11. Ensures that all objectives, for self and those directly responsible for, are clearly defined within the wider CityCare structures and in line with CityCare objectives, using the appraisal process as a vehicle for this. 12. Takes responsibility for their own and others health and safety in the working environment. Learning and Development 1. Undertakes specialist training required to develop and maintain their own and others proficiency in response to changes in clinical practice and emerging techniques, demonstrating high levels of competence within professional body requirements. 2. Reflects on and evaluates own practice and identifies areas of development by setting appropriate objectives via appraisal and clinical supervision. 3. Initiates and develops research and development programmes appropriate to clinical practice. 4. Creates and delivers specialist training and development programmes, within teams and to other services as appropriate. 5. Provides high quality, specialist advice, support and supervision related to clinical specialism to Nurses, other practitioners, agencies and individuals. 6. Ensures appraisals for direct reports are undertaken in accordance with Trust policy; manages performance which falls below standard in accordance with the appropriate HR policies and processes seeking support from senior staff where required. 7. Supports the ongoing learning and development of others including students, preceptees and apprentices. 8. Is an active mentor assessor for student nurses and an advocate for increasing student learner numbers in practice.
