Summary
Deliver personcentred, highquality care to patients in their own homes. Manage a complex caseload of highintensity patients with frailty, comorbidities and polypharmacy. Undertake communitybased clinical procedures including IV therapy, cannulation, phlebotomy, catheterisation and clinical monitoring. Make autonomous clinical decisions, seeking support appropriately. Work closely with GPs, Consultants and MDT colleagues to prevent hospital admission. Communicate sensitively with patients and carers, including in distressing or emotional situations. Support medication review and polypharmacy management. Maintain accurate, contemporaneous documentation using EMIS mobile. Promote clinical governance, audit and evidencebased practice. Contribute to UCR performance, KPIs and escalation management. Work in partnership with acute trusts, social care, care homes and voluntary services.
