Summary
As a Senior Community Nurse working with the Night Team most of the work allocated to you will of an unplanned nature. The service works as an on-call service covering the County of Worcestershire taking direct referrals from patients, 111, Ambulance Service and discharging hospitals. You can expect to have a comprehensive induction and work alongside peers until you are confident to then work doubled up with a Health Care Assistant. The Night Service nurses have work allocated via a handheld tablet by the triage nurse based at Droitwich. You will start your visits from the Droitwich Hub. As a band 6 you will rotate into the triage role based at Droitwich Health centre at least once a week. The triage nurse takes the lead for that shift and allocates the work to the team working across the county. The patients that are seen during the night vary immensely and no two shifts are the same. For example, we see catheters, palliative care, wound care, administration of medication, diabetics, and patients requiring intensive support rehabilitation following discharge from hospital or admission prevention. As a team we often undertake initial assessments of patient in the community. As part of a new initiative, we are working closely with West Midlands ambulance service and 111 and assist in responding to urgent Care response. These can be patients that have fallen, or patients requiring further assessment to aid prevention in admission to hospital. As a band 6 nurse in the team, you will have designated staff to supervise, and support. As part of this you will be expected to carry out annual appraisals for your designated staff members.
