Summary
Job Summary The post-holder will manage a clinical caseload and provide comprehensive medical services to registered and temporary patients of Western Road Medical Centre. The role is based in a busy primary care environment and requires the delivery of high-quality, patient-centered care while working autonomously and collaboratively within the multidisciplinary team. The post-holder will contribute to clinical governance, service development and the continuous improvement of care in line with contractual, professional and regulatory requirements. Clinical Responsibilities Undertake surgery consultations including face-to-face, telephone and digital consultations in line with practice systems Assess, diagnose and manage patients presenting with acute, chronic, complex and undifferentiated conditions Provide home visits as part of the Home Visit Rota Make autonomous clinical decisions and take responsibility for patient care Manage long-term conditions in accordance with practice protocols and national guidelines Provide preventative care, screening and opportunistic health promotion Develop, review and implement patient-centred care plans Provide counselling, advice and health education to patients and carers Refer patients appropriately to secondary care and other services Review and action laboratory results, clinical correspondence and referral outcomes Check, sign and issue acute and repeat prescriptions including undertaking medication reviews as appropriate Prescribe safely and cost-effectively in line with local formularies and NICE guidance Maintain clear, accurate and contemporaneous clinical records using the practice clinical system (EMIS) Collect and contribute data for audit, QOF and service monitoring Participate in the on call / duty doctor and on-the-day clinical workloads as rotad Other Responsibilities within the Organisation Work collaboratively with GP colleagues and the wider practice and PCN multidisciplinary team Share clinical and administrative workload equitably Attend and contribute to practice meetings and clinical discussions Participate in audit, quality improvement and service development initiatives Support teaching, mentoring and supervision activities where appropriate Maintain professional competence through appraisal, CPD and revalidation Comply with all practice policies, procedures and clinical protocols Please see attached full Job Description and Person Specification.
