Summary

CLINICAL Ensure medicines are stored, supplied and administered in accordance with Trust Medicines Code and NMC Guidelines for the Administration of Medicines and maintain own competence in this respect, reporting any learning needs to Ward Manager. Perform required clinical procedures (including specialty specific procedures) competently. Respond to health emergencies and contribute to the resuscitation of patients as needed. Inform the Ward Manager of any circumstances in which safe care cannot or has not been provided. In the absence of the Ward Manager act upon these concerns. Competently use clinical equipment, undertaking relevant training as required. Use IT Patient Information systems as necessary and in accordance with Trust policies, undertaking training as required. Ensure appropriate and timely communication with patients and relatives/carers, advising, instructing and supporting them as needed and making use of support systems as appropriate (e.g. interpreters, chaplaincy). Contribute to the development of patient information leaflets. DUTIES AND RESPONSIBILITIES OF THE POST HOLDER LEADERSHIP/MANAGEMENT Allocate work to junior members of the ward/departmental team ensuring this is completed to an appropriate standard. Ensure patients property is handled in accordance with Trust policy. Deal with any untoward incidents or complaints, in accordance with Trust Policies and Procedures and assist in their investigation and resolution in a timely manner. Contribute to the evaluation of clinical practice through audit. Attend ward meetings Ensure the ward environment is clean and welcoming taking steps to address any concerns and ensuring compliance with: Health & Safety Infection Control Fire Prevention Management Risk Management COSHH Act as a Link Nurse for a special interest area, as needed. EDUCATION AND PRACTICE DEVELOPMENT Participate in appraisal process, taking opportunity to identify and discuss own learning needs with appraiser. Support the induction of new staff (including bank/agency), taking on the role of preceptor. Demonstrate and teach nursing procedure to other staff, students (including post registration students) and patients as necessary. Participate in clinical supervision and act as Clinical Supervisor to nominated staff as required. Act as mentor to nursing students. Act as NVQ Assessor. Assist in developing and implementing innovative clinical and working practices, in consultation with senior nursing and medical staff. INVASIVE MONITORING Preparation and institution of arterial and central flushs and monitoring systems. Maintain arterial and central Flush systems in situ. Care of arterial catheters and central venous catheters. Perform zero checks and obtain blood samples for analysis. Interpret pressure monitoring tracings and troubleshoot problems. Interpret cardiac tracings and report appropriate anomalies. Perform haemodynamic measurements/calculations. To safety and competently attached the Lidco haemodynamic monitoring and interpret results and apply to treatment alterations as required. Titrate inotropic therapies according to prescribed parameters. Management and titration of multiple therapeutic infusions. To competently and safely perform those clinical extended skills to which the individual has undergone appropriate training and assessment for: Recording of ECGs Venepuncture Cannulation Male catheterisation The mixing and administration of intravenous antibiotics and a variety of critical care medications. To competently and safely manage patients who have epidural or PCAS infusions. To assist with and care for the patient undergoing insertion of chest drains. HAEMOFILTRATION Establishing renal haemofiltration therapy as prescribed by clinician. Care for patients whilst undergoing renal replacement therapy. Modify regime according to clinicians directives. Troubleshoot equipment problems hardware and consumables. Discontinue/suspend equipment/therapy according to local protocols. PATIENT TRANSFERS Organise and co-ordinate the transfer of critically ill patients according to Divisional guidelines and by liaising with other ICUs, bed bureau, Paramedic/ambulance staff and any appropriate medical staff. Transfer critically ill patients to intra-site (e.g. CT scan, MRI, Radiology and rehabilitation areas) and inter-site destinations maintaining all invasive/non-invasive monitored parameters ensuring comprehensive and accurate handover to any receiving nursing staff. Competently caring for critically ill paediatric admissions during stabilization period and whilst waiting for specialist PICU retrieval team to take over care of the child. CARE OF THE DYING PATIENT/ORGAN DONATION Support the patient/family and medical staff when caring for the dying patient and during any decision processes regarding withdrawal of treatment of a patient. Ensuring clear channels of communication exist thorough out the multidisciplinary team and significant others when implementing or evoking a Do Not Resuscitate order. Assist the medical staff in the procedures to determine whether patients have suffered brain stem death. Initiate and co-ordinate the process of organ donation when appropriate Liaise with the Transplant Team and participate in the organ donation process following brain stem death. Support the family during the organ donation process involving the Transplant Co-ordinator as appropriate. ENDOSCOPY Reprocessing and preparation of endoscopes. Perform safety checks and validate sterilisation status. OUTREACH Provide/co-ordinate outreach service/advice to other wards and departments when Outreach service is not available; within available resources. Participate in Outreach to other wards/departments when appropriate. Participate in the pre-operative preparation of patients undergoing major surgery by visiting and providing information in anticipation of elective ad EDUCATION /TRAINING Liaise with the Practice Development Nurse to: Support and teach new staff working through speciality-specific competencies as listed in the unit competency booklet. Assess and document speciality-specific competencies as they are achieved. Competently fulfil link nurse role, ensuring current research is communicated to ICU team, and undertake formal and informal training. Contribute to evidence-based guidelines as appropriate. PAEDIATRICS Liaise with multidisciplinary team regarding accepting an appropriate paediatric admission to ICU. Assess and implement a plan of care following discussion with anaesthetic and paediatric medical input. Liaise with PICUs to transfer patient to nearest specialist unit. Assist anaesthetist with complex procedures e.g. intubation, invasive lines, investigations etc Understand and be able to use paediatric drug formulas. Have a working knowledge of paediatric anatomy and physiology and understand normal values and be able to act upon the abnormal. Understand and be able to use specialist paediatric paperwork in conjunction with PICU departments and give a comprehensive hand over to retrieval team or accepting hospital.
