Summary


Cancer Pathway Tracker - 37.5 hours, fixed term 12 months

The post holder will provide essential administrative and coordination support to ensure patients on cancer pathways are monitored effectively and progress through their diagnostic and treatment milestones within national timeframes. The post holder plays a key role in supporting the delivery of high‑quality, timely cancer care by maintaining accurate data, tracking patient pathways, and escalating delays appropriately.

Working closely with clinical teams, MDT coordinators, managers, and diagnostic services, the Cancer Tracker is responsible for updating cancer systems, monitoring patient progress, and helping ensure that all tests, appointments, and results are arranged and recorded in line with national cancer targets.
• Monitor cancer patients across all tumour sites to ensure timely progression through the pathway.
• Update relevant cancer systems (e.g., Somerset, InfoFlex, or local databases) with accurate and up‑to‑date patient information.
• Track diagnostic tests, appointments, and results, ensuring they are recorded promptly and correctly.• Identify delays or risks of breaches against national cancer waiting time standards and escalate appropriately.
• Maintain high levels of data quality and support data validation exercises as required.
• Liaise with diagnostic departments (Radiology, Endoscopy, Pathology) to follow up outstanding tests or results.
• Assist in ensuring patients receive timely appointments in line with 2WW, 28‑Day FDS, 31‑ and 62‑Day cancer standards.
• Work with MDT Coordinators to ensure pathway information is available ahead of MDT discussions.
• Support the coordination of patient communication when required (e.g., letters, appointment updates).
• Communicate effectively with clinicians, nursing teams, booking officers, and operational managers to progress patient pathways.
• Provide administrative support to the wider Cancer Team as required.
