Summary


CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

We are seeking an experienced and motivated Advanced Clinical Practitioner (ACP) to join our Unplanned Care Services across South and West Hertfordshire. This is a fixed term Band 8a post for 12 months, covering maternity leave.

You will work as part of a highly skilled, multidisciplinary team, alongside Advanced Clinical Practitioners, a Nurse Consultant, nurses, paramedics, therapists, GPs, support workers and administrative colleagues. Our services are focused on providing timely, high quality care in the community, supporting patients to remain at home wherever possible and facilitating earlier discharge from hospital.

As a senior clinical decision maker, you will play a key role in supporting clinical teams and strengthening partnerships with internal colleagues, primary care, and nursing and residential homes. A central part of the role will be maximising appropriate referrals into our services through effective collaboration and relationship building.
Our Unplanned Care Services include:
• Rapid Response
• Early Intervention Vehicle (EIV)
• Discharge to Assess pathways
• Frailty Hospital@Home
• General Medical Virtual Ward

Our Virtual Ward and Hospital@Home services are delivered in close collaboration with colleagues at West Hertfordshire Teaching Hospitals NHS Trust.

In this role, you will provide advanced clinical assessment and decision-making primarily within the Rapid Response and Early Intervention Vehicle services, supporting residents to receive safe, effective care at home and enabling timely discharge from acute settings.

The post aligns with the four pillars of Advanced Clinical Practice. You will work closely with fellow Advanced Clinical Practitioners as part of a rota, providing robust advanced clinical cover across weekends and bank holidays.

Additional information:
• The service operates on a pre-rostered shift pattern, covering 8am–10pm, 7 days per week, including weekends and bank holidays
• The role requires travel across the service area; a car driver with access to a vehicle for work purposes is required (unless you have a disability as defined by the Equality Act 2010).
