Summary
The Band 8b Bowel Cancer Screening Clinical Endoscopist is a senior autonomous practitioner who providesexpert diagnostic and therapeutic endoscopywithin the NHS Bowel Cancer Screening Programme (BCSP). The post holder works at an advanced and strategic level, deliveringhigh‑quality, safe, and effective endoscopic care, while also providingclinical leadership, service development, education, and governanceacross the bowel cancer screening pathway.
The postholder will be skilled in both diagnostic and some therapeutic colonoscopy and will be expected to work across all screening sites with Calderdale, Kirklees and Wakefield. They will use expertise to maintain and improve the quality and consistency of health care experienced by patients and service userswithin both BCSP and symptomatic endoscopy services.
• Performing independent, high volume colonoscopy within both the BCSP and the symptomatic service, including endoscopic therapy appropriate to a successful candidates’ limitations
• Maintaining JAG and BCSP performance standards, ensuring maintenance of acceptable Key Performance Indicators (e.g. caecal intubation rate, SPA detection rate etc).
• Providing expert clinical decision making across screening and symptomatic pathways, including escalating care for the unwell patient, arranging further investigations and determining appropriate surveillance pathways
• Acting as a clinical expert and point of reference when complex cases and adverse events arise
• Work autonomously whilst performing screening and symptomatic colonoscopy on participants requiring IV conscious sedation. The post holder will also occasionally be required to work alongside an anaesthetist in situations where patients and participants require deep sedation or a general anaesthetic to support successful completion of their investigations.
• Appropriately maintain and update their clinical knowledge and skills and will use this to effect change appropriate to the on-going development and expansion of the National Bowel Cancer Screening Programme.
• Act as a lead within the clinical endoscopist team, overseeing and supporting the team’s development and compliance in all aspects for their roles.

• Have high level hand to eye coordination and a high level of manual dexterity to allow them to perform diagnostic and therapeutic screening and symptomatic colonoscopy, demonstrating techniques that prioritise patient safety and comfort.
• Be required to maintain moderate levels of sustained physical effort and dexterity during endoscopic examinations.
• Be required to maintain sustained levels of concentration during endoscopic procedures, using digital and chemical enhancement to identify and examine both benign and malignant lesions
• Wear appropriate PPE due to being frequently exposed to unpleasant conditions, including contact with bodily fluids.
• Be required to stand for long periods surrounded by equipment which can at times be noisy and generate a lot of heat.
• Will be required to use high-definition video display units, (using various wavelengths of light for digital enhancement of lesions) for sustained and prolonged periods.
• Ensure meticulous infection control measures are consistently applied to all areas of clinical practice, as defined by both national guidance and local policy.
• Will communicate effectively with various individuals from within the screening and symptomatic MDT, as well other professional groups, organisations, patients and their carers.
• Be able to communicate complex interventions, treatments, risks and outcomes to individuals, carers and families participating within the BCSP, commensurate with their level of understanding and language, respecting both equality and diversity.
• Understand and apply the legal safeguarding framework that supports the identification of vulnerable adults.
• Be skilled at delivering bad news to patients, families and their carers, ensuring that further staging investigations are requested and appropriate MDT referrals are made. The post holder will also ensure that patients and participants have access to on-going support, including named contacts, to allow further enquiries, questions and concerns to be addressed.
• Be able to demonstrate in-depth knowledge, understanding and experience of patients with lower gastrointestinal diseases and disorders.
• Interpret endoscopic findings, distinguishing benign disease from potential / actual malignant disease, instigating appropriate actions and management for timely further investigation and appropriate onward referral.
• Practice within their own limitations and escalate care where appropriate to ensure patient safety.
• Be an independent non-medical prescriber (post is open to those without such a qualification, however the desire to undertake such a course is essential).
• Work autonomously whilst performing screening and symptomatic colonoscopy on participants requiring IV conscious sedation. The post holder will also occasionally be required to work alongside an anaesthetist in situations where patients and participants require deep sedation or a general anaesthetic to support successful completion of their investigations.
• Ensure patients and participants can make informed and appropriate choices around sedation and analgesia.
• Be competent in the administration of IV conscious sedation, including the use of reversal agents and resuscitation equipment, should adverse events and complications arise.
• Be experienced in the recognition, management and treatment of lesions within the colon and rectum and will be competent in the therapeutic management of at least level 2 polyps, including the immediate management of post polypectomy bleeding.
• Refer large or complex benign polyps to a relevant site-specific Complex Polyp MDT, including an accurate description supported by photo documentation, and an assessment of likely complexity using the SMSA criteria.
• Manage non-malignant disease (e.g. IBD) that is detected at screening colonoscopy, ensuring that onward referral is directed appropriately. If the finding is acute and requires urgent attention, then the screening endoscopist will alert the appropriate medical / surgical team.
• Be able to recognise complications attributed to colonoscopy and will be able act on such findings appropriately and decisively.
