Summary

Coordinate the care of Diabetes patients and promote good practice in the diagnosis and management of CYP and their families. Provide advice and expertise on the management of children and young people who are living with a Diabetes diagnosis and their families. To provide CYP, families and staff with specialist advice to manage simple and complex symptoms. Lead child and family centred care and establish and maintain a supportive relationship with the child and their family/carers. Ensuring that the rights, dignity and confidentiality to the child are protected at all times. Following UHCW NHS Trust policy and guidance and on completion of the Diabetes Care CYP Course and Clinical Competencies, the post holder will titrate Insulin doses and undertake management of complex treatment options such as insulin pump therapy and to advise clinicians / parents / carers on the adjustments of diabetes medication. To manage nurse led clinics and baseline assessments, formulating clinical management plans. To undertake specialist investigations, interpret results and initiate appropriate action and to participate in Multidisciplinary clinics. The post holder will be able to provide emergency support to families at times of crisis such as illness management. To work collaboratively and share expertise within the multidisciplinary teams both locally and nationally. To act as a resource in the care of patients needs, ensuring an optimum level of service to patients and their families. To work closely with network, other hospital, and community staff to ensure an effective seamless service. To provide a high level of communication skills to enable effective and comprehensive care planning within the multi-disciplinary team. To build up a skill set to embed patient and public involvement within the sphere of specialist nursing practice. To work as a patient advocate, to help negotiate the patient journey and ensure optimum care is provided. To adhere to infection control guidelines and procedures. To act as a positive role model and promote an effective team ethos through the demonstration of excellent clinical skills. To promote health and well-being to CYP and their families with Diabetes and maximise opportunities for health education and promotion.
