Summary
To act as the Key Worker for a caseload of patients as agreed with the GI Cancer CNS Team Leader. To provide appropriate support, information, education, advice, and counselling with regard to all aspects of their illness and possible investigation/treatment modalities at all stages of the pathway. To act as the patient's advocate, particularly when informed discussion may lead to choices being made concerning investigation/treatment options. Assist in the process of attaining informed consent. Contribute to MDT meetings and treatment/care planning decisions. To work closely with other Health Care Professionals to ensure that all aspects of patient/carers needs are met, linking with palliative care and supportive care providers and social services appropriately. To continue to develop and liaise with other units/centres to ensure continuity of care and support for patients referred on for further investigations/treatment. Provide advice and support to health professionals within the Trust and Primary Care in relation to GI Cancer treatment side effects, complications and management and ensure liaison with appropriate members of the MDT.
