Summary
Core Activities Provide a full range of medical services as defined in the core GMS contract, additional and enhanced services where appropriate. Provide other personal medical services to meet identified patient needs. Make professional, autonomous decisions in relation to presenting problems whether self-referred, or referred from other healthcare works within the organisation. Assess the healthcare needs of patients with undifferentiated and undiagnosed problems. Screen patients for disease risk factors and early signs of illness. Develop care plans for health in consultation with patients and in line with current practice; disease management protocols, provide counselling and health education. Work within current ICB policy for prescribing. Maximise the effective use of current clinical computer systems. Record clear and contemporaneous consultation notes to an agreed standard. Compile and issue computer generated acute and repeat prescriptions, To take part in areas including health research, clinical auditing and data collection. Contribute to QOF requirements and recording data appropriately. Screening the patients for health education. Attend team meetings and other clinical meetings as necessary. In general, the post holder will be expected to undertake all the normal duties and responsibilities associated with GP working with primary care, as set by Royal College of General Practitioners, General Medical Council, all other Royal Colleges and Councils applicable to the staff within the Practice, Department of Health, IBC, CQC, and all other regulatory and standard setting organisations. Service Development Be involved in developing primary care services to meet the needs of the practice population. Working in close collaboration with the practice team to ensure that access targets are reached. Work with the practice team to ensure that immunisation and health inequalities targets as defined within the PMS contract are reached. Support health development and ensure health promotion work is undertaken to allow the practice population to make informed choices. Ensure the practice is represented at appropriate meetings, eg. Practice Based Commissioning Groups. Managerial Awareness of, and compliance with, all relevant organisation policies and procedures, e.g. prescribing, confidentiality, data protection, health and safety and infection control. Apply these to all aspects of one's own work. Committed to life-long learning and audit to ensure evidence-based practice. Contribute to evaluated audit and clinical standards setting within the organisation. Contribute to the development of computer based records. Contribute to summarising of patient records and read-coding patient data. Quality Alert other team members to issues of quality and risk. Assess own performance and take accountability for own actions either directly or under supervision. Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance and service expansion. Work effectively with individuals in other agencies to meet patients' needs. Effectively manage own time, workload and resources. Prescribe safe and effective medicines management which conforms to: NICE Guidance and DH directives. Good Prescribing Practice as defined by British National Formulary (BNF) guidance from Royal Pharmaceutical Society of Great Britain on safe and secure handling of medicine. Operate safe management and regular review for all patients receiving repeat prescriptions. Controlled Drugs Legislation. Shared protocols agreed between the ICB and other NHS providers. Locally specified Patient Group Directions. Working effectively so as not to exceed the Practice prescribing budget as set annually by the ICB. Comply with targets set within the Prescribing Incentive Scheme. Contribute to the team's approach which ensures that referrals are made for patients to see other medical service providers, ensuring that adequate information is given to personnel to whom the patient has been referred for proper assessment to be made. Be supportive of any complaints policy and procedure to ensure proper investigation and appropriate follow-up action in the event of a complaint being made about any aspect of the service provided or any personal provider of the service
