We are looking for a committed and skilled Adult Social Care Worker to join our integrated Recovery & Social Care Mental Health Service in North Somerset. The Role:
As an Adult Social Care Worker, you will:
• Work with adults with severe and enduring mental illness, promoting wellbeing, independence, and positive outcomes.
• Carry out Care Act 2014 assessments, reviews, risk assessments, and interventions. You will work closely with provider to review packages of care and ongoing care and support needs to ensure our service users are receiving person centred care in line with their identified needs to promote positive outcomes.
• Work closely with social workers on Mental Capacity Assessments, Best Interest decisions, and safeguarding activity.
• Contribute to Section 117 MHA aftercare planning and ensure statutory duties are met.
• Collaborate with a wide range of professionals, providers, and agencies to deliver excellent, integrated care.
• Review care packages, respond to concerns, and help prevent breakdowns in support.
• Work flexibly across community settings, service users’ homes, and office environments.
This role requires strong communication skills, resilience, and the ability to work independently while managing a diverse and meaningful caseload.
Key Responsibilities
• To provide strengths based Assessments of care and support needs and risks under the Care Act 2014 and S117 MHA, working collaboratively with service users and carers.
• Identify and respond to safeguarding concerns, contributing to both NHS and Local Authority enquiries.
• Support Mental Capacity Assessments and Best Interest decision-making alongside senior staff.
• Deliver a full range of statutory and social care tasks, offering specialist knowledge within the integrated team.
• Complete timely reviews in line with North Somerset policies, procedures, and statutory frameworks.
Who We’re Looking For
You will bring:
• Experience working with adults with mental health needs.
• A strengths based, person centred approach.
• Confidence working independently and making informed decisions.
• Knowledge of key legislation: Care Act 2014, Mental Health Act 1983, Mental Capacity Act 2005.
• Sound assessment skills.
• Excellent communication, problem solving, organisational skills and IT skills .
