Summary
The Care Coordinator will play a key role in supporting the delivery of high-quality, patient-centred care at Swan and Forest Surgery. The post holder will work as part of the multidisciplinary team to coordinate care for patients, particularly those with long-term conditions, complex needs, or who are at risk of hospital admission. The role focuses on improving patient outcomes by ensuring care is proactive, well-organised, and responsive to individual needs. The Care Coordinator will help patients navigate the healthcare system and ensure effective communication between services. Key Responsibilities Care Coordination Coordinate and support the development and delivery of personalised care plans Identify patients who would benefit from proactive care coordination Monitor and follow up on patient progress, ensuring appropriate interventions are in place Patient Support Act as a first point of contact for patients with care coordination needs Assist patients in accessing appropriate services Provide information and guidance to support patients in managing their health and wellbeing Promote self-care, independence, and patient engagement Multidisciplinary Working Work collaboratively with GPs, nurses, pharmacists, social prescribers, and other healthcare professionals Liaise with external organisations such as community health services, social care, and voluntary agencies Participate in multidisciplinary team (MDT) meetings and contribute to care planning Administration and Record Keeping Maintain accurate, timely, and confidential patient records Support recall systems, audits, and reporting requirements Use clinical systems to track patient care and outcomes Ensure compliance with data protection and confidentiality policies Quality Improvement Support initiatives aimed at improving patient care and service delivery Contribute to population health management and targeted interventions Assist with identifying gaps in service provision and suggesting improvements
