Summary
Surrey Downs Health & Care
An exciting opportunity has arisen for a motivated Rehabilitation Assistant (Band 3) to join the Stroke Supported Discharge Team within Surrey Downs Health and Care. This role supports stroke patients in their recovery and safe discharge from hospital by providing rehabilitation in patients’ homes and community settings, helping them regain independence and improve their quality of life.
Working as part of a multidisciplinary team, you will support Physiotherapy, Occupational Therapy, and Speech and Language Therapy programmes by carrying out delegated treatment plans, monitoring progress, and providing feedback to therapists. This is an excellent opportunity for someone passionate about person-centred care, community rehabilitation, and making a real difference in patients’ recovery journeys.
• To provide rehabilitation support services to the Stroke Supported Discharge pathway.
• To undertake specific skilled support work to aid the neuro-rehabilitation of patients in the Community. Treatment may occur in the department, patients’ home or other appropriate setting. Treatment may be provided for Physiotherapy, Occupational Therapy or Speech and Language Therapy.
• To carry own patient caseload, assessing and treating routine patients within guidelines agreed by the treating therapist. Monitor progress and make minor alterations to patient treatment programmes as required.
• Work unsupervised reporting back patient progress and informing the treating therapist of any problems. Receiving guidance and advice on an almost daily basis.
