Summary
Duties and Responsibilities Working autonomously within agreed practice protocols, local and national guidelines and PGDs, the Practice Nurse will deliver a high standard of patient-centred care across a wide range of clinical areas. Clinical Responsibilities: Management and monitoring of patients with diabetes Supporting patients with long-term conditions including asthma, COPD, hypertension, CHD and CKD Conducting chronic disease reviews and developing patient-centred care plans Delivering treatment room services including wound care, immunisations, cervical cytology. Undertaking health promotion and lifestyle advice to support behaviour change Providing cardiovascular risk assessment and management, including lifestyle and medication compliance support Delivering sexual health services, including contraception, family planning and advice Supporting womens health (cervical screening, menopause advice, continence) and mens health awareness Carrying out clinical investigations (e.g. venepuncture, ECGs, urinalysis, BP monitoring) and acting on results appropriately Administering and supporting immunisation programmes (childhood, travel and seasonal) Providing wound care assessment and ongoing management Supporting anticoagulation monitoring and safe management of patients on warfarin Delivering care under Enhanced Services, including health checks and minor illness/injury support Responding appropriately to urgent presentations within the practice Patient Care & Communication: Promote patient self-management and shared decision-making Identify safeguarding concerns and act in accordance with policy Recognise and adapt to barriers in communication Professional & Team Responsibilities: Maintain accurate, contemporaneous clinical records in line with NMC standards Work collaboratively with the wider multidisciplinary team Participate in audits, quality improvement and service development Maintain compliance with infection control, CQC standards, and practice policies Ensure confidentiality and information governance standards are upheld General Responsibilities: Prioritise workload effectively and respond to urgent needs Participate in mandatory training, professional development and revalidation Contribute to team cover during periods of absence Support continuous improvement and innovation within the practice
