Summary
We are looking for a dynamic forward thinking & motivated DSN or Dietitian to help develop and support the diabetes service for Barts Health. The post holder will be a team player with vision, drive and enthusiasm. The successful candidate will have demonstrable experience and proven clinical skills as a diabetes specialist nurse or dietitian.
The ideal candidate will have strong nurse/dietitian-led outpatient clinic experience in Type 1 services and either a working knowledge or a willingness to learn the new and developing diabetes technologies (insulin pumps; CGM and Hybrid closed loop).
In partnership with members of the multi-disciplinary team the post holder will develop, implement and evaluate a seamless specialist service, ensuring that patients receive the highest standard of clinical care. They act as a specialist and will demonstrate a high level of expertise within the specialist service, providing specialist advice, education and support to healthcare professionals and patients. The post holder will actively undertake clinical audits and research within the specialist area.
To be a point of contact and clinical expert for patients, relatives and clinician colleagues in conjunction and with the full support of the clinical team. The post holder will be expected to ensure that the patient receives the highest possible standard of evidence-based care in line with current NICE/JBDS guidance.
· To provide a world class service for patients with type 1 diabetes (and a small proportion with T2DM), including patients using insulin pumps and sensors. The roles includes pump starts and upgrades (with support of pump and sensor companies).
· To care for a cohort of highly complex patients with excessive morbidity and mortality in an area with high health inequalities, language and cultural barriers and low healthy life expectancy.
· The post holder may help care for pregnant woman with type 1 diabetes who are using an insulin pump and sensors.
· The post holder will be involved in our established transition service and will be an important member of the team and contact for some of our Young Adults.
· As a registered nurse you will be expected to consistently meet the performance standards as set out in the Code of Professional Conduct (NMC 2002).
· The post holder may be required to work across the Trust at any time throughout the duration of his/her contract. This may entail travel and working at different hospital sites.
