Summary


Our friendly team are looking for a highly skilled professional to join our team.

Where possible you will have experience as Palliative care CNS and is also non-medical prescriber, however training is available if you need to develop these skills.

A passionate commitment to supporting palliative and end of life care in hospital settings is essential. Practice innovation, self-motivation and excellent communication skills are vital to this challenging role. We require a dynamic, confident nurse leader who, as part of a multi-disciplinary team, can influence practice by demonstrating excellent clinical leadership and motivational skills.

The Palliative Care Team provides multi-professional palliative care services for patients cared by Southend Hospital. The Team has highly developed collaborative relationships across primary, secondary and voluntary care.

As the Macmillan Palliative Care Clinical Nurse Specialist (CNS), the post-holder will play a critical role in supporting patients and their families who have complex needs. This includes working alongside ward and clinical teams in all adult settings ensuring that individualised care for patients is consistently of a high standard that both coordinates and delivers equitable holistic care.

To carry caseload of hospital inpatients who require specialist palliative and complex end of life care nursing intervention, providing both support and management for complex needs.

Act as an information and advice resource for clinical teams on the care and management of patients and their families who have palliative and end of life needs.

Provide expert advice on symptom control, palliative and end of life care issues to staff throughout the Trust.

Act as a leadership role model for excellence, empowering others through education, support and the demonstration of expert knowledge and understanding around palliative and end of life care.

Participation in delivery of education on all aspects of speciality for Trust and South East Essex locality as required.

Demonstrate a high level of clinical expertise, to support and facilitate high quality individualised care to patients/carers in collaboration with the multi-professional team, and in line with local and national targets.

Identify and support Trust wide service and practice development needs for generic palliative and end of life care.

As an NMC registered practitioner provide independent prescribing within role (Palliative and EOL care).

As part of nursing rota, provide 7 day palliative care face to face service delivering first line CNS led weekend service.
