Summary
This strategic, multi-agency approach ensures patient-centred care, and reduces unnecessary Emergency Department attendances by providing safe and effective alternatives. The ICC Hub Care Coordinator will play a vital role in the day-to-day operation of the ICC service, coordinating the activity of the service to ensure patients receive a timely response. They will be responsible for overseeing case flow through the hub, ensuring timely and accurate data collection to support reporting and quality improvement activity, taking phone calls from clinicians as a point of access into the service, and supporting the onward referral of patients, as directed by the senior clinical decision makers and manage the flow of requests coming through the shared ICC inbox. The coordinator will assist the clinical operations manager with hub operations, including rostering, updating internal and external rostering systems, tracking and reporting clinical incidents via Radar, and managing LAS staff communications as directed.
