Social Worker Level 2 – West Community Wellbeing Team

We have a number of vacancies in our Community Wellbeing Team covering North and South Lincoln.

What you’ll do
Work closely with adults and families to provide tailored support and care planning in a strengths based and person centred way, embracing what community assets and strengths.
Champion independence, dignity, and wellbeing in every interaction
Collaborate with health professionals, community partners, and colleagues to deliver joined-up services
Bring compassion, resilience, and creativity to complex situations
Help develop our workforce through supervision of up to 4 Community Care Officers.
