Summary

Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

We are looking for a proactive and compassionate Health and Social Care Coordinator to play a central role within Adult Community Services in West Kent. You will act as a key point of contact for patients, carers and professionals, often supporting complex health and social care needs. You'll work across a wide range of partners—including Acute Trusts, Primary Care, Social Services and voluntary organisations.

You will support housebound patients living with long‑term conditions to improve their wellbeing and maintain independence. You will help reduce unnecessary hospital admissions by responding to referrals, offering advice and support, and signposting to other services where appropriate. You will manage your own caseload, planning outcomes and expectations with patients and carers to enable them to manage their health needs.

Excellent communication, organisation and analytical skills are essential, as is the ability to build strong relationships across multiple services. Additional Working Pattern: 22.5 hours/ 3x7.5 hours Monday to Friday
