Summary

Support and improve repeat prescribing systems Conduct medication reviews within scope of practice Monitor and action prescription queries Support Medicines reconciliation for new patients and after hospital discharge Provide guidance to patients on safe medication use Take responsibility for audits, QOF, IIF, GMICB BeCCor prescribing initiative, safety alert monitoring and other prescribing initiatives. Support the clinical team in delivering high quality outcomes Liaise with community pharmacies and other healthcare providers Maintain accurate and compliant medicines related documentation Contribute to practice policies, safety procedures and continuous improvement work Work with community pharmacies, PCN pharmacy teams, and external providers to ensure seamless patient care. Maintain accurate clinical coding and documentation in line with practice policies. Governance and Safety Adhere to all governance requirements including data protection, confidentiality, and medicines legislation. Uphold safe systems for storage, handling, and disposal of medicines.
