Summary

The post holder will be a qualified mental health professional working as part of the Specialist Community Forensic Team. You will jointly manage a defined caseload of patients with complex forensic mental health needs, typically no more than 15 individuals due to the intensity of the role. You will oversee each patient's pathway from inpatient secure care through to community living, maintaining regular contact in both settings. Many individuals will return to the community under restricted sections of the Mental Health Act, requiring skilled, multiagency working and intensive MDT input both before and after discharge. The role involves delivering evidencebased, recoveryfocused and strengthsbased interventions, integrating risk management, substance misuse support, social inclusion, and support with accommodation, education, employment and daily living skills. You will be expected to build and maintain strong therapeutic relationships, including with individuals who may be difficult to engage, while maintaining clear professional boundaries. You will communicate complex and sensitive information clearly to patients, carers and professionals, and contribute to inpatient reviews, risk meetings, tribunals and other professional forums as required. The post holder will actively contribute to clinical governance, audit and service development activity, working collaboratively to shape and improve this developing service. You will also play a role in supporting and supervising colleagues, students and newly appointed staff, and will deputise for the Team Manager when appropriate. This role requires flexibility and the ability to travel across Somerset and the South West region, holding a full driving licence with access to a vehicle.
